  • Our services include speech pathology assessments, therapy, and group programs. These may be delivered in-person, via telehealth, or at partnering schools.

  • Bookings and service delivery are governed by our Service Agreement, Cancellation Policy, and Consent Forms, which are required before therapy can begin. These documents are available through our online forms hosted securely via Splose.

  • Telehealth services are subject to internet availability and may vary in effectiveness depending on individual circumstances.

 

3. Fees and Payments

  • Service fees are outlined in the Service Agreement and follow the current NDIS Pricing Arrangements.

  • We work with NDIS participants who are self-managed or plan-managed only.

  • Payments may be made directly, or invoices will be issued to your plan manager. If your plan does not cover the services, you are responsible for payment.

  • As a non-registered provider, we are not bound by NDIS pricing limits. Our fees may exceed the standard NDIS rate, and you may be responsible for any difference not covered by your plan. This is similar to a gap fee when accessing medical services under Medicare.

 

4. Forms and Consents

The following forms may be requested from you during intake:

  • Client Intake Form

  • Service Agreement (NDIS and Private)

  • Cancellation Policy Acknowledgement

  • Telehealth Consent Form

  • Consent to Share Information

  • Feedback and Complaints Form

 

All forms are securely submitted via Splose and handled in accordance with our Privacy Policy.

Note: These Terms reference the key aspects of each form. Full legal details, such as responsibilities, rights, cancellation conditions, and authorisation to release information, are covered directly in the forms. Including the same content here would duplicate material unnecessarily.

 

5. Privacy and Data Collection

  • Personal data submitted through forms, emails, or bookings is collected in accordance with our Privacy Policy, and stored using secure third-party platforms (e.g. Splose).

  • By using our services, you consent to this data being used for healthcare delivery, communication, and invoicing purposes.

 

6. Intellectual Property

  • All content on this website—including text, graphics, logos, and downloadable materials—is the property of Little Gumnut Speech Pathology and may not be reproduced without permission.
